Initial design
Because I wanted to use the ITL200 on the microscope also, went towards more universal solution (Ray, thank you very much for the illuminator and sorry for the cruel surgery )
Camera version
Microscope version (could be used with the turret version with shorter objectives), kind a Optiphot infinity conversion
Objective dovetail adapter
ITL200 can be easily swapped between both versions (if needed)
ITL 200 adapter

Narrow end downwards, like in the your drawing at the right ?Don't forget, large end towards the objective, or large end downwards for best results.

When I had Nikon MXA20696 and no 3D printing was available for me - very succesfully used M42 extension tube and few layers of the black electrical tape. Initially I made it for testing purposes, but it served for the rest of the time ... For one end (do not remember, it was for Morfanon or Nikon20696) you could use piece of the orange Walgreen's medicine pills vial, fits perfectly between tube lens and M42 extension ring.

FYi, There is a DIY trick if you have a spare Sigma LSA. You can unscrew the retainer ring and remove the glass and mount the ITL200 or MXA20696 in both normal and reverse. The retainer screws back in and holds the lens firmly in place. If I remember correctly it only grabs a few threads so be careful.

Also RAF camera does have a38mm x 0.5 to M42/M52 adapter.
