Oops!
I didn't look very closely when I opened the envelope, and had forgotten it was one chip to start with. They're not quite as bad as Rik's, but nevertheless in 10 pieces!
The envelope didn't look badly damaged, but it's curved a bit like it went through a roller during sorting.

I'm not sure why your TLU has a small dovetail, unless Olympus made one for the BH-2 series (without the "U" prefix maybe?) I don't know about. With my BHS, I use an NFK 3.3x along with the U-PMTVC 0.3x adapter for my Panasonic GH4 micro 4/3 camera on a C-mount.
